Long before the internet began obsessing over his latest transformation, Randy had already been candid about the health struggles that completely changed his life. According to PEOPLE, the music producer was diagnosed with type 2 diabetes in 2000, years before he dramatically changed his lifestyle.
At the time, Randy weighed more than 350 pounds and admitted he never expected the diagnosis to happen to him, despite his family history. During a 2013 appearance on "The Doctors," he revealed, "It always happens to somebody else, even though it ran in my family, my dad had it. I never thought it would happen to me."
In his book "Body with Soul," Randy described the terrifying reality of that moment after learning his blood sugar was over 500. "I shouldn't have been shocked to learn that I had diabetes, but I was," he wrote. "Yes, I was carrying about 350 pounds on my five-foot-eleven frame, and both my parents had had the disease, two factors that put you at risk for type 2 diabetes."
The diagnosis ultimately became a turning point. Randy called it "both a blessing and a curse," explaining that it served as a "huge wake-up call.

After that day in the ER when my doctor burst the bubble I'd been living in, I couldn't lie to myself anymore," he wrote. "Right then and there, I began my journey toward better health. I had to."
In 2003, Randy underwent gastric bypass surgery, a decision that dramatically altered his life and appearance. Years later, he reflected on the process during an interview with PEOPLE. "For a long time, you see me on TV and people have noticed me in the past, and I was struggling with my weight, then I finally got it off by using some pretty drastic measures," he said.
The surgery helped Randy lose more than 100 pounds, but he later admitted the procedure alone was not enough. Maintaining the weight loss became an entirely different challenge.
"You lose a bunch of weight really fast, but maintaining's another thing because you get there and your mind tells you, 'Okay, phew. I'm here now. I can start to party and bring out the cheesecakes,'" he told PEOPLE in 2022.
Over the years, Randy repeatedly used his platform to raise awareness of diabetes and promote healthier living. In a 2018 Instagram post shared for World Diabetes Day, he reflected on living with the condition for nearly two decades.
"Since I was diagnosed with type 2 diabetes back in 2000, I've been sharing my story with others every day, but especially on #WorldDiabetesDay," he wrote. "Shout out to @Colgate Total whose unique formula that prevents and reverses gingivitis, for getting real with me about why a healthy smile matters," he added.
He continued, "Blessed to share some things I've learned over the past 18 years with others who have diabetes #ColgatePartner #DiabetesMonth."
Randy later explained that focusing on nutrition, gut health, and balanced eating helped him maintain his transformation. He also incorporated Pilates and spinning into his routine as he launched his nutrition brand, Unify Health Labs.
"I think if I had not figured it all out, I would've gained at least half, if not more, of that weight back," Randy told the publication. "And I'm proud to know that we did something that helps me, that helps everyone else, and takes the guesswork out of it for everybody."
Although Randy’s weight-loss journey has been public knowledge for years, his newest 2026 photos and videos still caught many fans completely off guard.
