Among the first features that caught the surgeon's attention was Brady's hairline. Dr. Levine suggested it appeared noticeably fuller than would typically be expected for a man approaching 50, noting that natural recession usually occurs regardless of genetics.
"No matter how superhuman you are on the football field, hairlines recede," the professional explained. "Even in people with great hair genetics, their hairline recedes."
He continued by pointing to the apparent absence of temple recession, adding, "That temple recession you see as men age is not necessarily an unattractive look. It doesn't necessarily detract from their aesthetic at that point. But in Tom's case, that recession has essentially disappeared."
Dr. Levine went on to say that Brady's exceptionally full hairline appeared almost too perfect for someone his age. "If you have someone who's nearing 50, whose hairline looks too good to be that of someone in their late 40s, no matter who you are in terms of genetics, it creates a disconnect," he said.
"So, having too good a hairline, which he has for his age, it looks unnatural. It looks kind of off. And so, there's no question that his hair has improved or changed," the surgeon continued.
Beyond his hair, Dr. Levine also suggested Brady's smile may have been enhanced through cosmetic dentistry, believing the former quarterback's current teeth looked significantly different from those seen earlier in his career.
The surgeon also pointed to the former football star's forehead, cheeks, and lower eyelids, observing an apparent lack of wrinkles and crow's feet despite decades spent competing outdoors in sunny climates. Dr. Levine suggested Botox was a likely explanation, noting:
"One of the signs of having a heavy brow is that you have to use your forehead muscles a lot more to elevate that brow. The only way you wouldn't do that is if there's some neuromodulator involved. So something like Botox definitely appears to be in use here."
He also questioned Brady's smooth complexion, saying the former quarterback showed little visible sun damage despite growing up in California and spending years playing football in Florida.
"Overall, his skin has kind of a great shine to it," the surgeon shared. "For a guy who grew up in California, likes being outdoors and spent time in Florida, he doesn't visibly have any real photodamage."
He suggested Brady may have undergone treatments such as IPL, BBL, or laser resurfacing to maintain his complexion, estimating that repeated cosmetic maintenance could cost tens of thousands of dollars annually.
Despite those observations, Dr. Levine did not believe Brady had undergone extensive facial surgery. Instead, he suggested the former athlete's fuller cheeks were more likely the result of fillers than surgical implants.
"Yes, he probably is in better shape and weighs less, but if someone is that drawn and loses that kind of facial weight through the combination of aging and maybe weight loss, you would expect the cheeks to be really quite drawn," Dr. Levine said while comparing recent photos with Brady's NFL headshot from 2001.
Instead, the professional noted that Brady's cheeks still appeared relatively full. "You don't really see that. In fact, his cheeks look quite full and look pretty good. It would be reasonable to think that he has kind of volumized that mid-face area," he stated.

"I think that after he retired, he may have done a lot in a short amount of time. It definitely makes it a lot harder to keep things natural."
