For years, Simon Cowell — the music mogul and television titan behind "American Idol", "The X Factor", and "America's Got Talent" — was as famous for his changing appearance as he was for his famously sharp tongue.

But in a rare moment of candour, Cowell has since admitted that the needles and fillers went far further than they ever should have — and that walking away from them was one of the best decisions he ever made.

'I Looked Like Something Out of a Horror Film'

Cowell, now 66, has not always been coy about his relationship with cosmetic procedures. For a man who built his career on blunt honesty, he has been refreshingly upfront about what he put his face through — and why he eventually pulled back.

Speaking to The Sun, Cowell acknowledged that there was a period when things spiralled. "There was a stage where I might have gone a bit too far," he admitted. "I saw a picture of me from 'before' the other day and didn't recognise it as me."

He went even further in the same interview, describing how he had come to look, in his own words, like something out of a horror film. But it was not a mirror or a magazine that finally snapped him out of it — it was his son.

Cowell's child Eric, who was eight years old at the time, began to grow visibly frightened by his father's appearance. For Cowell, that reaction was the jolt he needed. "Eric was in hysterics," he recalled. "Enough was enough. There is no filler in my face at all now. Zero."

In a more recent interview with The Sun, published in December 2025, he was equally blunt when asked directly about his cosmetic history.

"I used to have filler but then one day I saw a picture of myself and thought, 'Oh God, I look like a real weirdo,'" he said. "So I had everything dissolved."

A History of Tweaks — And One Dramatic Turnaround

Cowell's journey into cosmetic enhancement was not an overnight affair. In 2018, he publicly admitted to undergoing a nonsurgical procedure — a so-called "sewing bioplastic-infused thread" facelift — describing it as painful but effective at clearing up sun damage and unclogging the skin.

By his own account, Botox became a fixture over many years. "There was a phase where everyone was having their faces pumped full of this and the other," he said.

When The Sun's journalist Clemmie Moodie asked him point-blank how many units of Botox he had received over his lifetime, his answer was typically theatrical:

"Oh God, thousands."

Yet for all the procedures, Cowell has consistently denied ever going under the knife for a traditional facelift — a claim that has attracted considerable scepticism over the years given his dramatically altered look at certain points. "Everyone always thinks I have but I haven't," he insisted.

The shift away from fillers coincided with a turbulent chapter in his personal life. In August 2020, Cowell was involved in a near-fatal electric bicycle accident that left him with a broken back in three places.

Speaking about the crash's aftermath, he revealed that he had genuinely feared he might never walk again — and that depression had crept in during his long recovery. He even considered therapy as he worked to rebuild both his body and his sense of self.

By 2022, the music mogul — then 62 and visibly slimmer — had quietly moved away from fillers altogether as he entered what he described as a new chapter in his life.

How Cowell Looks After Himself Now

If Cowell once leant on injectables to maintain his appearance, his approach today could not be more different. The transformation has been striking — and he attributes it almost entirely to overhauling his lifestyle from the inside out.

The most dramatic change has been dietary. Cowell has cut out sugar, dairy, red meat, and gluten — a far cry from what a dietician once told him was the worst eating plan they had ever encountered.

He now eats largely the same meals every day: a green protein smoothie with fruit, blueberries, porridge, and tea for breakfast, followed by half a hot cross bun for lunch.

Dinner is taken at around 4:30pm, and because he tends to wake at 9:30 or 10am, he effectively practices intermittent fasting for up to 17 hours at a stretch.

He has also slashed his overall calorie intake to under 1,000 per day — with the occasional indulgence factored in. He does allow himself four low-calorie beers daily, though he pours half of each away to make a shandy.

When asked whether he had tried weight-loss injections, Cowell was dismissive:

"No, I have never been tempted, sorry."

He has also had a LINX band fitted — a medical device designed to prevent acid reflux — but was clear this was for a health condition rather than weight management.

On the exercise front, Cowell trains or cycles every single day. He does push-ups using a pair of small bars, completing somewhere between 300 and 600 repetitions in a session — and up to a thousand when he is really pushing himself.

He can do 40 in a single unbroken set. When Moodie cheekily suggested he demonstrate the feat on the spot, he declined with characteristic wit, saying, "No, Clemmie, I am not doing press-ups in McDonald's."

Sleep has become a priority too. Cowell gets around ten hours a night and swears by blue-light-blocking glasses to help him wind down.

He has also ventured into the world of biohacking. He is currently taking peptides, including NAD+ — a coenzyme associated with cell turnover and longevity — and reports feeling notably better for it.

He also uses a nasal spray daily and says he cannot remember the last time he caught a cold. His broader philosophy has shifted towards simplicity. "For me now, a lot of it comes down to healthy eating and drinking tonnes of water," he said.

The results have been visible. Cowell shed three stone and, by multiple accounts, looks better than he has in years.

Some of that, observers have noted, is likely tied to a settled and contented private life — he is engaged to his long-term partner Lauren Silverman, and has spoken warmly about the grounding influence of fatherhood.

He has also trimmed his professional commitments, though he returned to the judging panel of "Britain's Got Talent" in 2022 and remains very much active in the industry.

In characteristic fashion, Cowell has not abandoned all ambition when it comes to his physical future. Asked how long he hopes to live, he did not hesitate:

"I think I can live to 100."

"All he needs to do is clean up his look," a third offered, more measured in their assessment. "Doesn't look like himself. Went way too far," a fourth concluded bluntly — a verdict Cowell himself would likely not argue with.
