The legendary action star brought along the woman who has quietly stood beside him for over a decade, and once people got a good look at her, they had a lot to say.

Arnold Schwarzenegger, 78, stepped out with girlfriend Heather Milligan, 51, at a climate event in Vienna, Austria, on June 16, 2026, and what followed was a wave of comments that couldn't help but point out a shocking resemblance.

Schwarzenegger Brings Star Power To Austria

The former Governor of California attended the Austrian World Summit, an event tied to his own Schwarzenegger Climate Initiative, held this week.

He was joined at the event by former US Vice President Kamala Harris and Austria's President Alexander Van der Bellen, with the group posing together for photos as they arrived.

Milligan, a physical therapist based in Los Angeles, has spent the better part of 14 years staying almost entirely out of public view. She made an exception this time, appearing by Schwarzenegger's side in support of his climate work and giving photographers a rare, clear look at the woman he has been with since 2012.

Their Relationship Actually Started After a Big Surgery

Schwarzenegger has spoken about Milligan before in a 2023 People interview, and the story behind their meeting has nothing to do with red carpets or mutual friends.

The two met in 2012 after the actor needed shoulder surgery while preparing for "Escape Plan," the 2013 film he shot alongside Sylvester Stallone, who happened to have his own shoulder surgery on the very same day.

With only three months to get back into fighting shape, Schwarzenegger asked his surgeon for a referral, telling him he needed to be ready for fight scenes opposite Stallone.

The surgeon pointed him toward one person specifically, Milligan, explaining that athletes were consistently sent her way because she understood their training in a way few others did.

Once his recovery was wrapped up and filming ended, Schwarzenegger called her to say thank you over lunch. From there, as he put it, one thing led to the next.

His Relationship With Maria Shriver Today

During the same People interview, Schwarzenegger also addressed his continued closeness with ex-wife Maria Shriver, 70, who filed for divorce a decade before the split was finalized in 2021.

Despite the years that have passed, the two still coordinate regularly when it comes to their grown children, Katherine, 36, Christina, 34, Patrick, 32, and Christopher Schwarzenegger, 28, including showing up together for Patrick's 30th birthday celebration a few years ago.

The pair are also grandparents now, sharing granddaughters Lyla, 5, and Eloise, 4, and Ford, 1, through Katherine and her husband, Chris Pratt.

Schwarzenegger explained that even with separate lives, he and Shriver stay in constant contact about holidays, birthdays, and other family milestones, adding that there is no reason for him to feel anything but love toward her.

But the woman now by his side is Hilligan, and the decision to step forward now, even briefly, is exactly what set off the reactions online.

The Look That Had Fans Doing A Double Take

At the summit, Schwarzenegger kept things simple in a black suit paired with a white buttoned shirt, no tie, and black trousers. Milligan, meanwhile, wore a tailored, light mustard yellow double-breasted blazer with prominent gold buttons, layered over a matching silk camisole that featured delicate lace trim along the neckline, finishing the look with classic white trousers.

But it took getting a clear look at her face, not her outfit, that really got people talking. On Instagram, commenters zeroed in on who she reminded them of.

"Doesn't matter how old she is but she reminds me a lot of Maria except with blonde hair [sic]," wrote one person, while another added, "Exactly like Maria💙💙💙💙💗."

A third commenter put it even more bluntly, "Omg! She is the blonde version of Maria exactly." The comparisons didn't stop there.

Over on Facebook, people kept making the same connection. "Yikes!!! Thought it was Maria, at first," one comment read, while someone else simply stated, "She resembles his ex wife Maria."

Not everyone focused on the comparison, though, as one person said, "She doesn't look that much younger 🤷🏼‍♀️," pointing out that the 27-year age gap between them wasn't so evident.

A Woman Who Cares and Has Her Own Career

Schwarzenegger has been candid about why the relationship has lasted. In his 2023 interview tied to the release of his book "Be Useful: Seven Tools for Life," he described things with Milligan as fantastic, pointing to how much they have in common and how independent she is.

He has also praised her work ethic, noting that she put herself through college and earned a master's degree in business while working jobs like bartending to cover student loans.

According to Schwarzenegger, she grew up tough and became the driving force in her own family, caring for both of her parents during separate battles with cancer roughly two years apart.

That caretaking instinct apparently never faded. At the time, Schwarzenegger mentioned that Milligan had recently returned from a trip to Nebraska, where she helped look after her grandmother, and that she still called her grandmother every night for at least 30 minutes. He said moments like that are when you really learn what someone is made of.

Milligan has since built her own career well beyond the clinic where they first met. She now runs Elite OrthoSport, one of the largest physical therapy practices on the West Side of Los Angeles, treating clients that include NFL players, NBA stars, and UFC champions.

Schwarzenegger has admitted he likes stopping by just to chat with them, joking that being around elite athletes sometimes makes him feel like, in his words, "a little chicken."

The Vienna appearance wasn't the first time the couple stepped out publicly together, especially in regard to the actor's charity initiatives. They were previously photographed in St. Peter's Square in Vatican City on September 30, 2025, while attending a press conference titled "Raising Hope for Climate Justice."
