On November 5, 2021, as the "Sicko Mode" rapper performed, a large crowd surged toward the front of the stage and caused a mass accident, leaving nine attendees dead and more injured.

The Houston Fire Chief, Samuel Peña, revealed that the panic began around 9:00 p.m., and there were nearly 50,000 individuals who attended the music festival. He said:

"At the start of the mass casualty incident, when it was triggered … We transferred 17 patients to the hospital."
